LEVEL 1 - A 15 Page Person Centred Plan template, with headings, supplied to you as a PowerPoint document ready for you to complete with your own personal details. In addition, as part of this package, you will also receive Templates for setting out Aims and Objectives, and the details of the individual's Proposed Package of Care. The Level 1 documents are also available as paper copies which will be posted to you for an additional printing and postage cost of £6.00. LEVEL 2 - The same 15 page Person Centred Plan template supplied as in the Level 1 package, but it is upgraded and comes ready to use with suggested wording already in place, supplied as a PowerPoint document for you to edit and amend to fit your individual requirements. Also included are the Aims and Objectives and Proposed Package of Care templates, with suggested wording already completed.
LEVEL 3 - A Person Centred Plan in the same format as in Levels 1 and 2, but completed with your personal information after consultation with you, and also including completed Aims and Objectives and Proposed Package of Care documents.
